What a scam! Shame on you!!! Do you not have any integrity? Doctor’s note without even speaking to or seeing a physician. Such a money grab and LOW INTEGRITY. I only did it as a test to see how easy it was to get a FAKE doctor’s note, as you give to students for NO REASON! I hope you sleep well knowing you scam employers and schools with FAKE notes!

Put the wrong date signed I want a refund

Hi Jared, after you posted this review, we reached out to you by email to fix the issue with the dates and waited more than 24 hours, but received no reply. We had also not received anything from you before the review. The dates on every note are the ones entered by the patient in the intake form. We display them on screen and ask the patient to review them before payment, once on the dates step and again on the final review screen. We don't change any dates on our end. Even when a honest mistake slips through, our support team usually sorts out reasonable date-change requests within a few hours. Our support inbox is still open to help. Under our policies, we accept modification requests within 72 hours of an order. We are still here for you.

Each one includes the physician's NPI and state license number, which can be looked up publicly with a simple Google search, along with a QR code and a Document ID that can be confirmed at sickslip.co/verify. Our focus is on providing a legitimate, verifiable document for workers and employers, rather than on how it looks. My work did not accept my note and gave me a written warning.

Hi there, Thank you for taking the time to share your feedback. We genuinely use every review to improve. I'm sorry your conversation with your HR director ended in a warning. When you wrote to us that your "note did not work" and you got a warning, we asked what the reason was, so we could try to address it — you told us she "would not expand" on the reason. We then offered two things to help anyway: a supplemental letter from me (the physician who reviewed your case) and a direct call from me to your employer to walk them through verifying the note. Both offers still stand, at no extra cost. We firmly stand behind our service — every note is reviewed and signed by a licensed physician, with verifiable credentials and a verification portal your employer can use any time.
