I started out saying that i needed a form filled out, and somehow someway you fumbled that statement the whole way. The doctor had no clue each visit we had. I informed the doctor of what i needed he said it wasnt his job contact support. Well support was fully aware before the appointment was even scheduled. Even the second appointment the doctor had no clue. I went as far as explaining to the doctor the whole ordeal. And finally he was able to locate the document in question. Overall horrible experience the doctor was great but he had no idea what i actually needed. Need to work on communication between support doctors and patients.

You changed my provider without notifying me. My state then notified me that I have to fill out a new form. Other than that everything else is good.
